Here at Patford House Partnership we take the responsibility for looking after your personal health record very seriously. All our staff members are trained on up to date GDPR guidance and work to strict confidentiality guidelines
Our Privacy Statement
We take your privacy seriously and have a strict code of conduct when it comes to patient confidentiality. New General Data Protection Regulations, which came in to force for Primary Care in May 2018, provide our patients with more control over who can access their records and what kind of information we hold in their medical notes. Personal Health Data
This practice may supply personal health data to comply with its legal obligations from time to time, as directed by the Secretary of State for Health, or other recognised Statutory Authority. Anyonymised data on the use of fit notes is being provided to the HSCIC on behalf of the Department of Health and the Department for Work and Pensions. This will enable to the Department of Work and Pensions to undertake research analysis to inform policy relating to employment and sickness absence, including evaluation for Fit for Work.
Fair Processing Notice
Hospital attendances are obtained from the Health and Social Care Information Centre (HSCIC) and matched to our data to create a "risk profile" about you. This is used to tailor appropriate services around your patient needs. All information will be handled confidentially and kept secure.
You have a right to object; please let the Practice know in writing if you wish to opt out of sharing your data, but please be aware this will restrict some aspects of "Risk Stratification" and "scoring" a patient's risk of being admitted to hospital. For more information click the above icon
Your Summary Care Record
A Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had.
Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.

You may wish to opt out of having a Summary Care Record created and maintained for you and you should let us know if this is your choice. In order to opt out from this service please download, complete and sign this form. Please hand your completed form to Reception who will be able to help you accordingly.
You can view a live summary of your own health record through your Systm Online or Airmid account.
Better Data Means Better Care
Patients in our area may have received some information in the post about Care.data which is a data extraction service commissioned by NHS England. The data which is extracted will be used to help NHS England to improve services, promote health research and to provide evidence of trends and other statistical information.
