We are always happy to welcome new patients to Beversbrook Medical Centre. However, all patients will be required to fill out our registration form that can be picked up at our Reception desk at anytime during opening hours and our friendly Reception staff will be happy to answer any questions you may have and the Registration Champion will contact you in process. You will be required to complete our Registration Form and New Patient Health Questionnaire and these are available from the Reception Desk.  Alternatively, you can start the registration process through the following link,


Completing an application on-line will initiate a registration request only.  There is still a requirement to visit Reception to bring in a copy of the registration form, to complete other documentation.


Please note that applications for registration have to be approved and refusal of applications may occur for example if inadequate information is provided on the registration form, you are out of our Practice Boundary or if you are a serving member of the armed forces.


It does take up to three weeks to process registrations.  We cannot offer patients any routine appointments before this time.

Practice Boundary

We can only register patients within our contracted practice boundary which covers the Calne area.  The PDF map below shows this boundary in more detail. 


You will note that the map shows a second outer boundary area also.  This area is for existing patients who move from the contractual boundary area into the outer boundary.  These patients will not need to re-register with another GP Surgery.  However, new patients will need to be living at an address which is inside the inner boundary line (our contracted boundary).  For further clarification or for more details please contact Reception.
